MY POINT-WISE EXPLANATION

MY POINT-WISE EXPLANATION
$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$$
3-12-07
Increment and Bonus
You have been warned so many times by the undersigned about your attitude to work properly and improve your performance.
Of late, you have committed serious lapses and shown lack of interest in carrying out routine jobs.
1) On 10-10-06, STG tripped while carrying out “On load valve test” on high pressure Emergency stop valve and subsequently GTG #3 & HRSG #1 & #3 tripped. Upon investigation, it was discovered that wiring termination of ESV trip coils were interchanged by you which resulted the unit trip and huge financial loss to the Company. You have been severely warned by the undersigned for the above lapses committed by you.
------------------------------
Further to above, I would like to clarify that:
The Dept Manager had assigned the job a Technician to carryout in night shift; and it was executed by in night. Then why the warning letter to me? What is the relation with me?
I had never changed any wiring. Was there any enquiry on the issue? What is the base of acquisition neither I carried out nor I assigned? Moreover, when it was executed or when the trip occurred, I was not present in the plant. Actually I was cornered as SCAPEGOAT based on others input.
How can I be held responsible?
If you are so particular about the punishing on the mistakes - May I ask - what action has been taken …………… when:
On 13 March 2007, when GT1 was ready to start after the outage, a Senior Manager has blown 4 Nos. of Power Supply Units of Gas Turbine 1, estimating more than 90000 Rupees, due to complete negligence on his part.
Even a layman would have stopped before blowing off the Redundant Power Supply Unit, thus running the GT.
WHAT WAS THE ACTION TAKEN THEN?
Apart from the cost of the equipment, the negligence has incurred a loss of more than 7000 MWH (approx. 60 MW per Hour x 120 Hrs.), as GT1 Could not be started for more than 5 days, it was started on 18 March 2007 and Synchronised at 12:31 Hrs.
Why was NO ENQUIRY? What way the inventory of 4 Nos. of power supply managed?
Instead, you compelled the PCE & PSM not to record the fact in the Log Books and Daily Report as well …………?
When the smallest accident or miss-happening in the plant is recorded as incident report, WHY WAS NO REPORT PREPARED FOR SUCH A MAJOR DISASTER?
Even the Departments and Plant Monthly Reports were manipulated to hide the fact with just one little sentence “FOUND TO BE DEFFECTIVE”.
· How could some running equipment be found defective?
· Has it ever happened to any other similar Gas Turbine or similar Power Supply Unit?
· Why did you safeguard some one in one case and injudiciously punish to other?
I am sure; even a person without professional and specialised knowledge would have stopped when the 2 Units did not work.
This is just one incident I have quoted; there are several of these kinds.
Even during the DCS migration to OVATION, there are several trips on part of C&I wrong, unskillful and quite nonprofessional decisions. Those could have been very well avoided with little intelligence.
I am sorry to say; for all those you have simply closed your eyes and never tried to find out the actual facts, WHY?

2) On 23-11-06, SPGL refused to offer appointment letter while changing over from Rolls Royce to SPGL. Basing on your promise that you would improve your work performance/qualities, an opportunity was given to you and that too with your immediate Manager’s recommendation.
------------------------------
I remember the evening when SPGL was issuing the Contract. You called me in your room and said that Mr. Ravishankar has telephoned you, not to take me in SPGL.
I fail to understand the reason, why would Mr. Ravishankar telephone you for not issuing me the contract. Even today, he would not know me personally.
You started clarifying that you did not intend my dismissal, and started giving unnecessary rather superfluous clarification.
You also said that you called Mr. Ravishanker and tried your best to convince but he was not at all ready to listen.
I realised the draft of the plot. Hence without speaking any thing on the subject, simply sought your permission “Shall I go now?” and left with your affirmation.
Later on I came to know that you are ready to issue the contract provided I agree to work with Operation Department?
Now the fact of Mr. Ravishanker telephoned you for not taking me in SPGL was very much transparent and free from the doubt.
Actually, you wanted to favour someone you feel very personal and privileged him to head the department therefore nobody would be there to fill the gap.
------------------------------
3) Subsequently, due to your very poor performance and negligence in discharging your responsibilities, you have been transferred to Operation department from C&I department as punishment. Unfortunately, still you have not changed your attitude towards working with the system.
------------------------------
As I have mentioned, shifting me from C&I Dept. was not a punishment. Rather, TO FACILATATE YOUR PRIVILEGED PERSON, SO THAT THE ENTITY OF YOUR FAVOURITE WITH SPGL COULD BE ASSURED.
I am from C & I Field and have worked for more than 30 years. My knowledge about the System & Power Plant is no way less than any body in the C&I Department today. Though I am better than many in the plant, whether it is C&I or Operations, still you have degraded me for your personal grievances.
------------------------------
4) On 27-11-07, after switching of the Pretreatment Plant Transformer #1, you changed the 3.3 KV breaker selector switch of Main Cooling water pump instead of Pretreatment Plant Transformer #1 from “service” to “removable”. Had you changed you changed the position of some other breaker, any of the units or Station would have tripped.
------------------------------
Above mentioned is not absolute truth.
The Shift PSM planned to isolate 415 V Switchgear first by Shift PCE + Technician and then we 3 will remove 3.3 KV Switchgear.
To monitor the smooth opening of both the breakers, Shift PCE & Technician were at 415 V Switchgear Room, whereas, it was myself alone at 3.3 KV Switchgear.
It was 06:25 Hrs. and the 415 side was not completed. It would be too late for writing the Shift Logs by PCE & PSM.
I removed the breaker alone and locked the door. While shifting the trolley loaded with such heavy Breaker, the trolley touched the CW Pumps Service Switch and an alarm was initiated in CCR at 06:28 Hrs. the mistake was rectified immediately. Just to clarify, the rubber sheets on the floor makes too difficult to trolley, especially by single person.
But my intention was fair and all that happened was un-willingly and un-intentionally, that too, to facilitate the PCE & PSM writing the Shift Logs.
Where as, without analyzing the fact you accused me of possibility of TRIPPING the whole STATION.
It may very well be confirmed from the drawing, as per the schematic, EVEN FOR RUNNING PUMP, IF THE “SERVICE” POSITION CHANGES, IT WILL result in an ALARM.
HENCE, IT IS TOO HYPOTHETICAL TO IMAGINE the STATION TRIP.
------------------------------
This is serious lapse in your duties and the responsibilities as a senior engineer. This shows your lack of concentration and interest in the job.
------------------------------
I agree the lapses, but my intention was fair, I tried to be loyal to the Company and to the Shift PCE and PSM and took additional interest on the job.

I am intelligent enough to assess the consequence of failure. If there would have been any possibility of Station or Pump, I would never take such step.

------------------------------
5) Initially, you joined in Operation department, because of poor performance, you have been transferred to C&I department again you were transferred to Operation department for the same reasons, but once again you failed.
------------------------------
I was removed from Operation Department as you were Operation Manager and personally did not like me. I remember and can quote several instances in support but so far I could not establish the reason – WHY?
And if, it was proven that my performance is poor, in November 2006 why was I sent to Operations Dept again?
Moreover, despite the fact that I have been removed as punishment from C&I Dept., recently, in October 2008, again you have transferred to C&I Department.
I do not think that it’s too difficult to understand the reason.
Actually you want to humiliate me and show your power for your personal reasons and gains.
------------------------------
6) THERE IS NO 6TH POINT IN YOUR WARNING?
7) Your annual performance appraisal by your Department Manager is very poor.
------------------------------
Could it not be your pressure on the Managers? Rather one of the Managers has confessed that he is helpless in this regard. I concrete evidence / proof for.
------------------------------
Therefore, your salary increment and bonus will be NIL for the year 2008. Further, you are hereby warned that any serious disciplinary action including dismissal will be taken if you repeat any such mistake in discharging you r duties and/or lack of interest in your duties or lapses committed in your duties again.
------------------------------
I am fail to understand the reason; why do you personally dislike me and have always demoralised me. Did I personally harm you?
Maintaining personal enmities would never allow taking the decision in favour of SPGL interest. Company’s interest will automatically be at second.
In my opinion, since I am paid by the company, my loyalty must always for the company first.
Hence, I hereby sincerely request to reconsider and revive the Discriminatory / Disproportional Punishment.
